Hi Arild,
Yup, it's refraction plus altitude. The "ground" is dropped for
both effects (set a ridiculously high altitude such as 10000 km,
for example, and you'll see the "ground" drop to cover a 46-degree
circle at the nadir.) This helps ensure that the rise/set times
match the times the objects actually reach the horizon.
