Lawrence, about the MPC report file "header": see

The file you'll be editing is 'header.mpc'. Some example details
are given in that file, which you should modify to match your actual
observer details.

An addendum to Bob's comment on display of MPC observations in Guide:
as he mentioned, you can right-click on an observation symbol and get
"more info". The date/time of the observation will be highlighted;
click on it, and Guide will reset to the date/time of that observation.
If it's a good observation, and you have current orbital elements for
the target object, that object ought to suddenly appear next to the
observation symbol. Yet another good way to check that all is well
with your data.
